PHASE 1

 

Phase 1, is called “Assess and Define,” and activities include two critical assessments: 1) Organizational Readiness and 2) Community Health Needs. We’ll work with you to conduct surveys, focus groups, and site visits to collect all the data. Once the research is complete, you’ll have enough evidence to guide your decision for the best program model to deploy.  The final deliverable of this phase is a detailed business case that includes your assessment data, risk analysis, cost estimates, and tangible goals. PHASE 2

 

Phase 2 is Develop and Plan. We’ll help you put together a detailed project plan for your program and technology that will serve as a roadmap for clinical and technological implementation. We also include performance monitoring and quality improvement plans to maximize your longterm success. PHASE 3

 

Phase 3 is Implement the Project Plan. To stay on track, we’ll compare the baseline schedules, budgets, and resource requirements to the actual progress to ensure all activities are carried out according to the plan. Heather holds a Bachelor of Science in Business Administration, with a concentration in Finance from the University South Florida, Tampa. She also earned a Master of Science in Health Informatics from Walden University, Minneapolis, MN. Heather is a member of the Florida Telehealth Work Group, the American Telemedicine Association, and the Pinellas County chapter of Better Living for Seniors.
